Dear Artist, Parent and Guardian,

Welcome and great to have you onboard! During your time with My Friendly Planet you will be able to explore, drawing, painting, collage, and other art forms and techniques. You will be mainly studying under me, Tina and other qualified artists. Together we will support you to experience an enjoyable creative journey in an enviroment which celebrates individuality and community, well-being, passion for the arts, learning new creative approaches and skills. You will be shown how to safely use tools and equipment. Anyhow, look out for our upcoming classes, workshops, projects and events as we will be offering various creative aspects found in the Creative Industry for all ages and levels.

*Anyone under 18 taking part in the project must provide a consent form completed by a parent/ guardian. Participation in the project becomes active upon booking, and completion of this form.

Failure to inform us of any existing medical condition puts users at risk, including allergies; we accept no liability. Please tell us about any medical conditions and/or allergies here:

I am aware of the risks involved in taking part in My Friendly Planet ’s Art Class/Club/Project/Workshop/Wex and I have sought medical advice for any ailments/medical conditions which may put me at risk whilst participating.
I have declared any existing injuries and medical conditions to My Friendly Planet staff member, and I am fit to participate.
I accept the risks involved in participating in My Friendly Planet Art Class/Club/Project/Workshop/Wex and for my own safety I agree to follow all instructions given to me by any member of staff.
I accept that I am responsible for my own actions and/or involvement.
I give permission for medical assistance to be given in the event of an accident or emergency.

Acknowledgment of Risk

Safety
1-Further safety guidelines can be found below. All users should read the whole of this document
2-It is recommended that ALL participants participating in an MFP workshop/class/programme/club must wear satisfactory safety equipment including goggles, breathing apparatus if required
3-It is required that ALL participants take part in safety use demonstrations of any tools, materials and equipment before working on their project
4-It is recommended that ALL participants safely use any tools, materials and equipment when working on their project
5-Anyone suspected to be intoxicated or under the influence of drugs will not be allowed to enter or remain in the My Friendly Planet Art and Design School venue of work
6-Smoking/Vaping is not permitted on the premise.

Equipment
7-My Friendly Planet does not accept responsibility for the loss, theft or damage to valuables or personal property of participants.
8-All use of creative equipment must be used only after a safety use demonstration. These include: cutting/craft knives, Lino but cutting tools, epoxy resins, acrylics and spray paints, glue gun, pliers, wire cutter, hammer, wires etc.

My Friendly Planet Art and Design School Etiquette
9-Be aware of other My Friendly Planet participants at all times – do not do anything that endangers their safety or yours
10-Do not hang out the windows
11-No sitting on stairs
12-No running, shouting or screaming
Bullying, abusive behaviour or violence towards staff or other users will not be tolerated.
13-You are expected to deposit any litter into the bins provided
14-If you notice any hazard or damage, you must report it to a member of staff. Vandalism will not be tolerated

Photography
–I give permission to My Friendly Planet to photograph or film me as a participant participating in a MFP workshop/class/programme/club or as a spectator. I agree that the photographs or films so taken may be used by My Friendly Planet for the purpose of promoting My Friendly Planet Art & Design School’s projects and activities .- You will have completed a photography/image consent form for this.
If I take photographs within the My Friendly Planet premises or event, I will be bound by the terms of the My Friendly Planet policy (available on the website).

Data Protection/ GDPR
My Friendly Planet does not sell, transfer or otherwise disclose the personal information given in this form, unless required to do so by law. Personal details are stored securely, and this form will be retained for as long as the individual named above continues to be an active participant in My Friendly Planet workshop/class/project/event/wex.
